  • In order to make your visit to our Clinic as efficient and pleasant as possible for all concerned it is advisable that you do the following: –

  •  

  • Please print and bring along any information you wish to discuss, to your next appointment.

  •  

  • Have your referring health practitioner/physician either send to the clinic directly or provide you a referral note.

  •  

  • Please bring along the accurate names of ALL  Medications (Prescription and Non-Prescription).

  •  

  • If at all possible please bring All Medications in the prescribed containers

  •  

  • Try to remember all significant health care related issues that you have encountered in your life as they may be relevant e.g. diabetes, hypertension arthritis etc.

  •  

  • Familiarize yourself with the basic costs of examination so that you may be prepared to settle your costs at the end of the visit.

  •  

  • Decrease your stress and cost, by filling out the appropriate Insurance Forms as are applicable.

  •  

  • Most major insurance forms are available on this website for downloading and completion, prior to your visit

  •         

  • Please bring along all current spectacles, contact lenses and/or prescriptions.

  •  

  • It would be extremely helpful if you were to fill out prior to your visit the Patient Questionnaire

  •  

  • Please be prepared to WAIT!  Most thorough  examinations require a minimum 30 minutes.

  •  

  • Please advise the office a minimum of 24 hours prior to cancellation or postponement of an appointment.

  •  

  • WARNING:    You may receive drops that will affect your vision. Sunglasses and an escort are strongly advised. It is highly unlikely that you  will be able to drive yourself home  safely after the Appointment!
